Athletics Registration Process and Forms 

All families who are planning on having their children participate in Christian Life School athletics must complete the registration information and return the forms to the CLS Office.  These forms must be completed before the first day of practice, as student athletes must be completely registered before they will be allowed to practice or participate in any way.  The registration information contains a summary of the general athletic rules that students and parents should understand.  If there is a question about any rule interpretation, please contact the Athletic Director. 

REGISTRATION INSTRUCTIONS

1. Print the Parent Student Handbook (.pdf, .doc) and read the following sections:

a) Communication Chain of Command (pg. 2).
b) Parent Coach Relationships (pg. 3).
c) Relationship with Game Officials (pg. 4).
d) Student Athlete Code of Conduct (pg. 4).  Must also be read by the student athlete.
e) Student Eligibility (pg. 5).  Must also be read by the student athlete.
f) Parent Code of Conduct (pg. 7).
g) Administration of Codes of Conduct (pg. 9).
h) Responsibilities of Parents (pg. 10).
i) Participation of Non-CLS Athletes (pg. 11).  Must be read by all Non-CLS families.
j) Registration Checklist (pg. 13).
k) Refund Policy (pg. 14).
l) Volunteer Requirements (pg. 20).

2. Complete the Registration and Payment Form using one of the following methods:

a) Method 1.  This is the preferred method where we want you to use the Registration Packet.  This packet is an Excel workbook which contains four worksheets.  The four worksheets are labeled as:

Instructions.  Read this first.  It provides step by step instructions on how to use this packet.
Registration Form.  This is the standard form used to register students.
 
CLS Student Payment Form.  This form will automatically calculate fees and deposits due for CLS students.  The registration form must be filled out properly for this form to accurately calculate fees and deposits.

Non-CLS Student Payment Form.  This form will automatically calculate fees and deposits due for Non-CLS students.  The registration form must be filled out properly for this form to accurately calculate fees and deposits. 

b) Method 2.  If you are unable to use the Registration Packet in Method 1 then print the necessary forms below and fill them out manually.
 
 Registration Form (.pdf, .doc).  Required to register all student athletes.
 CLS Student Payment Form (.pdf, .doc).  Use for CLS students only.
 Non-CLS Student Payment Form (.pdf, .doc).  Use for Non-CLS students only.

c) Method 3.  In the event you are unable to use Method 1 or 2 you will need to come into the school office where you can pick up the forms you need to register your student or you can use the computer in the main office.

3. Print and sign the required forms and turn them into the school office.  Bring the signed copy of the Registration Form and a copy of the Payment Form along with your payment, and any required deposits into the school office.
 
Note: the registration process is not complete until all of the forms and fees have been turned into the school office.  To avoid any late fees, all forms and fees must be turned into the office before any established late fee deadline. 

You may also mail the forms and fees into the school office.  Late fees will be assessed based on the post mark date.

4. Attend the Parent Student Meeting scheduled for Tuesday, November 8th, 2011, at CLS in the sanctuary @ 6:00pm.  Bring your printed version of the Parent Student Handbook (step 1) to the meeting.  Parents that do not attend this meeting are still responsible for the information provided at this meeting.
